What do I bring with me?

Bring the completed medical form that you will be sent with the confirmation email of your date, time and venue of your appointment. If you have information from previous investigations or orthotics, those are helpful to bring along too.

What should I wear?

Comfortable clothing that allows treatment of the affected area e.g. shorts for a knee, ankle or hip problem or a vest top for shoulder or neck pain.

What will happen at the appointment?

Helene will ask you questions regarding your symptoms and will assess and treat you at the first appointment. If you require further treatment or onward referral, that will be discussed at this point.

What happens if I am not sure of my condition or treatment?

Helene prides herself on taking the time to ensure patients fully understand their condition, and have the opportunity to ask questions and be fully involved in their treatment. You will be given options and advice at every contact so bring your questions with you!

What happens if I/my child needs podiatry?

Can I use my insurance?

Achilles heal is registered with the main insurance companies. You must always check with your policy provider first before commencing treatment. You may need to provide your policy number at the point of enquiry to allow your claim to be registered prior to your treatment commencing.

What information will be collected and how is it used?

The confidential information you have given will be retained by Helene Whitaker, Achilles Heal Physiotherapy for treatment only and will never be given to any other person without your consent or for any other purpose. You have the right to withdraw this consent and review or withdraw the information that is held at any time by contacting Helene Whitaker at helene@achilles-heal.co.uk. This will not affect your treatment in any way.

Retained Information will include your/your child's address, DOB, medical history, family history and social circumstances and this will be recorded by Helene Whitaker in your/your child's notes. This is held in accordance with CSP guidelines and the IOC with whom we are registered.

If you are providing consent for Helene Whitaker, Achilles Heal to treat and retain information for someone under the age of 16 year, you must have parental responsibility or be a legal guardian of the patient.

Your patient records will be kept for a minimum of 6 years and if you are under the age of 18 years, until the age of 25 or for eight years after final treatment in accordance with CSP legal requirements.
